I'm a solo developer. I take on a few projects at a time and do the whole build myself, including the boring parts most people hand off.
One person, the whole build.
No team, no account manager in between. You talk to the person who's actually writing the code.
I keep the workload small on purpose: a few projects at once, so each gets real attention while it's being built and after it ships.
